Prepare for changes to implement projects (#173)

* Remove EXT_CHANNEL from the configuration

* Remove EXT_OWNER_NAME

* Move generateId into its own file

It is useful in multiple contexts.

* Allow providing a linked user ID

* Set Content-Type headers

* Create a recursive-delete module

* Clean up the model files

* Remove a spurious error message

* Set the coordinator URL in the application

* Use local twitch-ext only in local mode

* Correct Windows "yarn start" command

* Improve start-up message

* Remove the login button from the navigation bar

Since it's not possible to run the Developer Rig without being signed in
to Twitch, the button is never visible.

* Move code in src/core/rig.ts to src/index.tsx

There are no meaningful tests for the code in src/core/rig.ts.

* Mock local storage item removal

* Remove unnecessary display Booleans

In all cases, the client component manages the visibility.

* Rename Booleans from verbs to gerunds

* Log unknown actions only if requested

* Get the coordinator URL from the outer HTML
