<a href="https://github.com/wikimedia/parsoid/commit/761c41850">Improve byte machine format.

Instead of being super-clever with implicit field width encoding, just
write it directly in the edge structure.  Also remove the &quot;anything
else&quot; equivalence map and just expand the &quot;anything else&quot; edges explicitly.
Use a [lower,upper) range format when writing edges so we take advantage
of adjacent character classes w/o requiring the use of an equivalance
table lookup.
