Reduce cert lookups within a single update (#193)

Clean up the certificate lookup code a bit and limit the number of
lookups to a single one per update. Before it did `n*2` lookups for `n`
ingresses.

Also fixes a logic error where certificates for multiple ingresses would
accumulate and later ingresses could potentially have more certificates
defined than needed.
