Add `distinct_repeated_events` argument to `create_pmap()`

For process map with many loops, it might be better to distingush the repeated events.
With this argument added, the occurance sequence number will be attached to the event name,
so the repeated event can be better visualized. It's very useful to identify
the events with `Kebab effect`.

* Fix `render_pmap_file()` format recognition bug.
