Final code push for CRAN (docs not final) (#54)

* Moved working environment helper functions to helper_functions file.

* Split fabricate.R -- each level function moved into own file.

* Fixed error in draw_normal_icc that incorrectly expanded cluster level data to individuals.

* Fixes for draw_normal_icc, functionality updates for draw_normal_icc, fixes for documentation, documentation examples, test updates (may impact coverage)

* Push roxygen docs to fix broken build.

* Grammar fix in docs for cross_level

* Abstracting the private arguments into helper functions, which closes #31

* Big pass on documentation, errors, argument names, and tests.

* Massive testing sweep to clear up coverage issues from last refactor and added functionality in draw_normal_icc

* Speedup of merge level index mapping.

* Neal's recommendation about double booleans; also added an edge case check to sd and sd_between being out of sync.

* Neal's recommendation to abstract sd/sd_between error checking into helper function.

* Additional test to normal ICC and fixed display of certain errors.
