Cleaner x-modules & config updates

Two improvements to x-modules:

- The key of each entry is now a path pattern as in `paths`. This documents
  the actual "mount point", offers namespace control for the parent scope, and
  supports overriding specific paths in derived specs using the yaml `<<`
  merge operator.
- Each path entry in `x-modules` is now an array. This lets us register
  multiple modules at the same mount point.

Also:

- Updated specs to reflect these changes.
- Refactored huge methods in router.js into smaller ones, and introduced an
  ApiScope class to consistently encapsulate scoped state during tree
  building.
