Reorganize and cleanup Jekyll Assets for the final release.

Fix #190: Created `jekyll/hooks` and `jekyll/assets/hooks`.
Fix #189: Move a lot of setup stuff into hooks/*.rb decoupling it.
Fix #187: Murder those overbearing specs and split them up more carefully.
Fix #188: Use `Hash#fetch` and create `Hash#fetch_or_store`.
Fix #185: Tests use `asset_path`, it seems to work.
Fix #168: Add specs to confirm aliases work.

This patch cleans up the source and splits up a lot of the env class into
decoupled hooks that add features.  It also moves internal proxies and other
stuff into `addons/` and organizes it in a way that we can split them up quickly
if in the future that is necessary.  It also organizes them.

* Move Jekyll's hooks into `jekyll/hooks`.
* Move internal hooks into `jekyll/assets/hooks`.
* Move around specs and make them easier.
* Make source easier to read.
