Browsable API improvements and bug fixes.

Upgrade to Bootstrap 5.
Fix bug where redirect routes and routes with subdomain constraints
would appear malformed due to parsing the routing formatter output;
switch to inspecting routes directly.
Group routes by controller and add drop-downs for all but the first
route group.
Put some helpers into a Utils module to prevent polluting rails
namespaces with RRF helpers.
