In dialogue mode, adjust programs produced by the exact match (#729)

Make sure that the output of the exact matcher is a dialogue
state, not a program, and that the statements are adjusted to
include all the implicit queries.

This avoids producing programs that are not valid and would
confuse the parser with unexpected slot fills.
