AppDV: NOW TRACKS VALID/INVALID FIELD STATES

Started new plugin 'appdv-field-plugin' which is applied to each field
object as it is created by AppDV. This plugin now hooks to the Field
'valid' and 'invalid' events to apply and remove CSS class names to the
field element, and the record element:

  * appdv-field-invalid (set/unset with Field)
  * appdv-rec-invalids (set with Field, unset after *all* invalids clear)

Added new CSS rules to disable record toggle save, and field edit save,
element/controls, respectively
