Improve logging interface

The logging interface has been improved, with the log writing functions
now supporting automatic addition of code location metadata for
diagnosing exceptional conditions.

A subtle bug in the plugin object implementation has also been resolved;
we were incorrectly passing an empty string to dlopen() which leads to
undefined behaviour.

The code layout has been improved further with the introduction of the
base sub-directory in the src directory.

This commit has been rebased from the issue/42/improve-logging branch.
