Split plugin logic into separate functions for ease of understanding

Rather than having long blocks of code all nested within the `hook.tap` callback, split them into separate functions for easier understanding.

This will also make the plugin more configurable both with users' webpack config files and with extra plugin options, including:

* User config: If the user passes in a function/promise into their webpack config's `entry` field, we can wait until it's processed before injecting mocks.
* Plugin option: If the user wants to choose which `entry` object key to inject mocks into (rather than arbitrarily choosing the first one).
* Plugin option: If the user wants to choose a `module.rule` to process mocks with.
* Plugin option: If the user wants to toggle the injection of the mocks dir into `module.rule` on/off.
