Update PagingIterator (#737)

* Add AsyncPagesIterator which is nicer to use than PagingIterator for most uses

* listItems

* Trying out a backwards compatible API.

* Refactor to hide nextPage storage.

Pave the way for construction without a response.

* No need to throw

* Remove duplicate file from tests

* Remove element buffering

* Document initializer. Rename nextPage.

* Remove initializer which throws away the first page of results.

* Small change to initializer

* Initialize paging iterators for all the relevant APIs.

* Fix tests after rebase

* Update docs with new pagination
