auth fixes to work with CB 3.x

Before this change, when cbft/cbgt was pointed at a 3.x Couchbase
Server, cbdatasource would return an error related to the
select-bucket command in cbdatasource (perhaps select-bucket isn't
implemented in 3.x CB?).

This change provides a GetSaslCredentials()-capable auth
implementation only when there's a non-empty AuthSaslUser parameter.

And, this change also moves the CbAuthHandler code out of defs.go into
its own cb.go file, so that defs.go can remain relatively clean of CB
related concepts.
