handle error in connect with useful error message

Previously, an error in the `DBI->connect` call would keep going,
eventually dying on the `$dbh->prepare` line with `Can't call method
'prepare' on undefined value`.

Now, we report the error from the database so the user can attempt to
address it.

Fixes #131
