Fix issue #8: don’t replace `Array#flatten`

Replacing `Array#flatten` with `Monad#flatten_with_monad`,
which we had been doing by default, seems to break
ActiveRecord relation eager-loading, at least in Rails 3
(see issue #8).

Therefore, we restore the original behavior of flatten
to prevent this surprising failure for Rails users.

If you prefer the previous behavior, it continues to exist
as `Array#flatten_with_monad`.
