EDUCATOR-5499: Prevent learners from submitting empty responses (backend) (#1569)

* prevent empty submissions and update some wordings

The frontend should catch all cases where this will happen, but there is a documented case where it did not catch an empty submission. We still aren't sure how it happened, but this is to catch it and prevent it from happening again.
