check for bankcross after each block

old one only checked occasionally (on incsrc, org, or end-of-file),
presumably to limit the performance impact of the check, but this makes
the errors much less helpful, and the performance penalty of the check
is pretty much nothing as it's just a few bitwise operations on 32-bit
variables.
