properly rewrite forwarded headers

there can be another proxy in front of sozu that adds its own forwarded
headers, so here is the behaviour we follow now (before, there was the
option to just let them through unmodified):
- X-Forwarded-Proto: if this header was present, keep it (it represents
the client's chosen protocol), otherwise add our own
- X-Forwarded-Port: same as X-Forwarded-Proto
- X-Forwarded-For: if it was present, rewrite it to append our own public
IP address, otherwise generate it
- Forwarded: if it was present, rewrite it to append our own values,
otherwise generate it
