Add measurement of the total elapsed time that ignores stopped time intervals

A progress bar can now measure the total elapsed time when a bar is stopped
and then resumed multiple times. The measurement will ignore stop intervals and
only accumulate running time. Also, the elapsed time is now measured inside
a progress bar instance as opposed to the formatters. All formatters will read
the exact same value which will guarantee a consistent display of elapsed
time and ETA.
