Commit transaction in field_lists function

Previously, field_lists only worked because a transaction was committed by correct_user_permissions.

- Make field_lists run concurrently
- Escape SQL statements properly
- Execute SQL statements individually
- Fix user/role typo
- Fix function docstrings
- Extract COLUMN_COMMENTS_SQL from _add_field_list_comments and docs_table_ref
- Rename SUMMARY_TABLES to SUMMARIES
- Rename SummaryTable to Summary
- Remove unnecessary set_search_path() call
- Rename methods and variables to be clearer
- db: Add and use format() method in execute() method
- tests: Test with and without --field-lists
