add skeletong of pmid system, work toward a better error taxonomy

the error taxonomy still needs a lot of work and it needs to be very
clear and careful not to follow the countless mistakes of http where
at all possible

also docs identifiers general writing, there was some extensive
writing on the error taxonomy that led to a realization that the
internet is completely inside out and that its insideoutness has
infiltrated into the identifiers that are associated with it, in
summary: How do you resolve a doi from Mars?
