Extract most configuration behaviour to Cucumber::Configuration.

Cli::Configuration should just be a thin adapter over Cli::Options.
We will always give a Cucumber::Configuration to the user in
AfterConfiguration.

@mattwynne - couple of things to take note of:

1. Passing `out_stream` on to the Cucumber::Configuration in
   Cli::Configuration#to_hash
2. Handling profiles properly
3. Cli::Configuration defines a log method, I think this should move
   too? But there's no test requiring it... yet.
