plugin: Convert to Grafana's ReactJS based plugin framwork

Migrate to grafana-toolkit and the ReactJS based plugin framework
introduced with Grafana v7.0.

This allows to drop a large amount of configuration settings because
we can simply inherit them from grafana-toolkit.

Fixes: #29, Fixes #31.
