session: disconnect excessive res-usage sessions faster

session.extra_cost() only 'works' after session_mgr.add_session(session) was called.
session.recalc_concurrency() uses extra_cost.
So previously, sessions that belong to a session_group that accrued a lot of cost
would only get disconnected if either (1) they themselves also accrued at least 100 cost
(trigger for recalc_concurrency inside bump_cost), or (2) during the periodic recalc
(every 5 mins)...

Now, the session will get disconnect at the latest when they send their first request.
