kernel: fix OpenAppend to actually set format flag

... and set it to TRUE to match OpenOutput. Previously, we did not set this at
all, and instead whatever value `format` was set to previously in the reused
buffer was used. That means that some code using OpenAppend might behave
differently depending on when and where it is called.
