Fix bug which could double-apply pfx (during reauth, etc)

Added check to ensure the pfx (aka mount_url) is only applied *once*
per request 'options' object (JavaScript side). Typically there is a
one-to-one ratio of options object to request, *except* in the case
of reauth, custom prompt, etc. This change fixes broken reloads after
reuath for apps which have a mount_url
