Add support for disabling mappings.

Currently, because the mapping progress is pruned if the mapping is not
configured, there is no way to pause processing. After the progress is
removed, the processing has to be restarted from the first row in the
DB. The patch adds a "disabled" option for a mapping, which is optional
and defaults to false. If a mapping is "disabled", it is skipped, but
otherwise its progress is unaffected.
