Commit Message

When previously connected slave reconnects, delete old instance.

...and instantiate the slave again and re-insert into the master's
bookkeeping system.

This fixes the bug/issue from
https://github.com/box/ClusterRunner/issues/198 that leads to the
service freezing up upon reconnectin an existing slave.
