Fixes #111.  decodeFirstSync now matches decodeFirst and throws an error if there is extra data.  The exception for both now includes a 'value' member with the value that was decoded. Cleaned up a few tests and the implementation of decodeFirst and decodeAll, which now both always return a Promise even if you pass in a callback
