Improve custom consumer documentation. (#120)

* update code example with pre- and post-processing examples.

* clarify header paragraph

* scale should not be required.

* model_name and model_version should be passed in through environment, not through redis.

* Improve Consumer docstrings.

* split(':') the model_name and model_version.

* move process functions into TensorFlowServingConsumer.

* Restructure into an ordered list of instructions.
