Fix #189: Prompt users to fill in messaging details

Only if they have never done it before (that the phone numbers are all
null in the appropriate setup fields), it should display on the main
page.

Issue #189: On the the Contact Info button, add the phrase "Text messaging
            not enabled" to encourage volunteers to set their mobile phone
            preferences.
