Refactor unhandled rejection handling

Change handling of unhandled rejections to not longer
force abortion. It is possible to rethrow those fatal errors
from a custom error handler which then effectively reverts to the
old behaviour from 2.x. The documentation has been adjusted but
recommends against rethrowing errors as exceptions.
