New method for indexing dynamic tabs

Using tab name to index allows finer control over open tab/close tab behaviour:
- When a tab is opened, show the new tab
- When a tab is closed, keep the same tab open (if it is not the one that was closed) otherwise show the tab at the end

Also some messing around with the formatting and layout of the two buttons. Not as pretty now but this could be fixed with a bit of effort (I am sure).
