Add support for `mongodb` v3

This change adds support for the current version of the `mongodb`
driver, whilst maintaining backwards-compatibility with v2.

This change largely follows the corresponding change in `sharedb-mongo`:
https://github.com/share/sharedb-mongo/pull/83/

All unit tests are now run against both versions of the driver to ensure
compatibility.
