Commit Message

Refactor initialization pipeline (#289)

Now the initialization can be done by a single call `start(rf, init)`.
`start` calls `initialize` when it hits the bottom reducing function.
This makes using stateful transducers with fold functions that are not
aware of Transducers.jl API easier.
