Simplify library initialization instructions

- make `init` call `init_worker` automatically if `init` is called from
  `init_worker`.
- add more details to the error message about counter initalization.
- adjust README to recommend initializing the library from
  `init_worker_by_lua_block`.
- use `_block` versions of all lua sections in the README.
