Fail pending streams when last sessions disconnects.

If all sessions in a pool closed, we weren't doing anything with the
pending streams. With no additional requests being queued, and no
reachability changes, these requests would have to time out.
Alternately, in poor networking conditions like airplane mode, pending
requests could just pile up, never getting dispatched onto a session
since the socket would always fail, and again staying queued until they
timed out.

This fixes that poor user experience by immediately failing all pending
streams (ones that have not been dispatched onto a session yet) when
the current session pool count goes to 0.
