Improve the skewed merkle tree implementation:

* More closely mirror the normal merkle tree implementation API
* Prefix leaf and node values differently before hashing them
* Implement gen_proof so you can generate a proof for a given value
* Simplify the structure of the proof, now it's a simple list of hashes
* Store the hash fun in the #skewed{} record rather than suppling it
  every time to ensure the tree is consistently hashed
* Do not store the leaf value in the tree, it's not needed
