Fix proposal bug (#13)

* add test that checks if the proposals used by the ptsampler match what is given

* bug fixes in last commit

* add mixed proposal test to bounded normal test

* fix serious bug in determining whether to use a default proposal

* only get known parameters in bounded normal

* allow bounded normal's __contains__ to work on a subset of parameters

* correctly format parameters for dictionary in proposals test

* add proposal parameters check to std changes test

* python 3: make sure joint proposal's parameters are set to a list

* make a copy of the given proposals dict when setting up proposals in samplers

* store parameters as a frozenset

* switch back to using tuple for parameters, but keep order

* whitespace

* fix for 2.7

* remove unused import

* update the example notebooks

* undo having bounded normal only pull out the parameters it knows about when
jumping; that is now handled by the joint proposal

* bump version number
