Fix broken elastic search and my workflows (#3321)


#3310

Attempts were being made to assign null to the Java primitive boolean
isLegacyVersion field in WorkflowVersion. This would happen when
fetching workflows, which would at least cause ES reindex
and My Workflows to fail.

Existing nulls were there because frozen version were unable to be
initialized. Update those nulls, and don't allow nulls in the table.

This branch also tackles #3253, as I needed it to debug migrations
problems.
