Move things around to avoid "class ActionTree.action.Action" in docs

Sphinx' add_module_names setting is global and I've found no way to cheat
and document class ActionTree.action.Action as "class ActionTree.Action".
Idem for exceptions.

ActionTree is small enough to put them in __init__.py but the same issue
on a larger package would be inconvenient.
