Handle `rd` single sign-on parameter properly

Closes #288.

Before, `rd` would always be set to the server name, discarding whatever value
already existed. This meant that accessing a non-root URL before
authentication always redirected to the root URL after authentication, e.g.
trying to access hub.18f.gov/foo/bar would eventually always redirect to
hub.18f.gov/.

With this change, the server name is set as the prefix to the existing `rd`
parameter, so the user is redirected to the original URL after authentication,
e.g. hub.18f.gov/foo/bar.
