fix frame extraction of videos with wrong container...

metadata.

Previously we introduced video_duration for video files whose duration
is missing or wrong in their container metadata. That didn't really work
though, since input seeking (-ss flag before -i) still can't seek to the
right place without the correct container metadata.

As a work around, we are now using output seeking (-ss flag after -i)
when video_duration is specified. This solution works, but it is
extremely slow, especially when each frame (total 16 by default) is
seeked from the beginning, so on average the whole video is decoded
roughly eight passes.
