Implement an addStream() method to be used with arbitrary streams.

Allow arbitrary streams to be added to the pipeline. It is not
currently safe for arbitrary streams to be intermixed with other
operations, so gate the functionality behind a flag that becomes
set as soon an addStream() is called that prohibits add() calls.
