nail down scopes and splices

This is a big commit with lots of changes.

- splice all primitive constructor, and combiner sub-templates
- add quote-template and unquote-template forms inline scope munging
- make "#/" read as "unquote-template" in #lang template files

Everything is documented and needs more tests.
