Model changes (#55)

* Removed issue mappings and added direct relationship on the digital object

* Added file set member mapping for mapping files to more than one digital object

* Added a on_delete nullify foreign_key_option to contained_by relationships

* Made desc terms nomenclatures and exemplary images direct relationships rather than polymorphic (see migrations)

* Added additional specs to filestreams (resolved #25, resolved #41)

* Fixes to Factory Bot sequencing/ authority migration to stop build from breaking (resolves #31, resolves #35)
