Remove subject model (#93)

* consolidate subject model into transcriptions

* delete files associated with subject model

* remove subject_id from transcriptions in db

* remove subject from transcription factory

* remove references to subject_id for transcriptions model, factory, serializer, and db table

* fix issues with tests

*  fix more errors in tests, create custom validator for checking json attribute is not nil
