Address overlap-save review comments

Some old comments by @ssfrr had not been addressed, mostly relating to
documentation matching the code. One more substantial comment was making the
overlap save algorithm work in situations where `nffts < 2 * sv -1`, in which
case more than the first "block" will require padding. I have addressed these
comments with this commit. Interestingly, this will also eliminate situations
where no blocks require padding (e.g. if `nv = 1` in a dimension).

In addition, I have changed some variable names in `optimalfftfiltlength` for
clarity, and also changed some tests which were using the old interface, which
used to have `nb` and `nx` reversed.
