Don't store user claims in a nested dictionary

This makes it easier to have control over the created token (easily
do things like modify the `iss` claim, etc).

Also, if a `@jwt.user_claims_loader` callback is present at the same
time that the `user_claims` arg is passed to create_access_token or
create_refresh_token, the results will be merged together.
